STUDENT MEDICATION

Florida State law requires that a special form be filled out and signed by the physician and parent/guardian of each student on medication.

This form must be on file in the school clinic before a student can take any medication at school.

All physicians have copies of this form.

This also includes non-prescription medication such as aspirin.

The medicine must be properly labeled with the original label and must be stored and taken in the school clinic.

Whenever possible, please only send those dosages that are to be taken during school hours (weekly basis).
