New Student and Kindergarten Registration

*To register more than one child ,you will need to use a new email address for each student enrolled.  You can use a generic email such as child1@home.com, child2@home.com, child3@home.com*

Registration

The following documents and information are required when registering a new student:

• A certified birth certificate. (Kindergarten students must be five years old on or before September 1, of the current year.)
• A document showing proof of residence such as a utility bill or lease agreement.
  • ​The Following can be used:
    • ​ documentation of homestead exemption
    •  a copy of lease agreement
    •  current rent receipt
    •  a copy of current power, water, or gas bill
    •  a statement from owner/lessee of the home where they are living, stating the names of all people residing at the address.  In addition, enrolling parent/guardian must bring a current utility bill (power or water) with the owner/lessee's name and address
• Florida Immunization Form (blue sheet from health dept. or doctor)
• Physical---(Cannot be over one year old, and must include the doctor's name and address if form is from out of state)
•  Copy of student's social security card

Note:

When custody of child(ren) is in question, we must have copies of parental custody documents to be kept in records folder. When guardian ship of child(ren) has changed from enrollment, we must have copies of notarized guardianship paperwork.
